ABSTRACT

Background/Aims@#To determine the value of salivary pepsin in discriminating sub-types of gastroesophageal reflux disease (GERD) and GERD-related disorders. @*Methods@#Overall, 322 patients with different sub-types of GERD and 45 healthy controls (HC) were studied. All patients took Gastroesophageal Reflux Disease Questionnaire (GerdQ) and underwent endoscopy and 24-hour esophageal pH monitoring and manometry. Salivary pepsin concentration (SPC) was detected by using colloidal gold double-antibody immunological sandwich assay. Oral esomeprazole treatment was administrated in the patients with non-erosive reflux disease (NERD) and extra-esophageal symptoms (EES). @*Results@#Compared to HC, patients with erosive esophagitis, NERD, EES, EES plus typical GERD symptoms, or Barrett’s esophagus had a higher prevalence of saliva and SPC (all P < 0.001). There was no significant difference in the positive rate for pepsin in patients with functional heartburn or GERD with anxiety and depression, compared to HC. After esomeprazole treatment, the positive rate and SPC were significantly reduced in NERD (both P < 0.001) and in EES (P = 0.001 and P = 0.002, respectively). Of the 64 NERD patients, 71.9% (n = 46) were positive for salivary pepsin, which was significantly higher than the rate (43.8%, n = 28) of pathological acid reflux as detected by 24-hour esophageal pH monitoring (P = 0.002). @*Conclusions@#Salivary pepsin has an important significance for the diagnosis of GERD and GERD-related disorders. Salivary pepsin and 24-hour esophageal pH monitoring may complement with each other to improve the diagnostic efficiency.

ABSTRACT

Objective To analyze the characteristics ofgastroesophageal reflux disease (GERD) under general gastroscope.Methods The detection rates of GERD related abnormalities such as esophagitis,Barrett esophagus and hiatal hernia under the first gastroscopy of the adult GERD patients from January 2013 to January 2017 in our center and the statistical relationship between the abnormal findings were analyzed retrospectively.Results A total of 4086 GERD patients,2004 males and 2082 females,were included in this study,and the age was 18-89(50.4 ± 13.3) years old.The detection rate of non erosive GERD was 78.7%,esophagitis 21.3%;non Barrett esophagus 87.7%,suspected Barrett esophagus 8.3%,Barrett esophagus 3.9%;generally normal cardia 61.4%,short segment hiatus hernia 20.4%,and long segment hiatal hernia 18.2%.The detection rates of esophagitis showed statistically significant differences (P<0.001) between the three groups of generally normal cardia,short segment hiatal hernia and long segment hiatal hernia;The detection rate of Barrett esophagus was significantly higher in long segment hiatal hernia group than in generally normal cardia group and short segment hiatal hernia group (P<0.001,P=0.012),but the difference between the later two groups was not statistically significant (P>0.013).Comparing the three age groups of 18-39,40-59 and ≥60 years old,the detection rate of hiatal hernia was significantly higher in the group of≥60 years old than in the 18-39 and 40-59 years old groups (P=0.007),while there was no significant difference (P>0.013) between the 18-39 and 40-59 years old groups.The detection rate of esophagitis was significantly higher in ≥60 years old group than in 18-39 and 40-59 years old groups (P=0.004,P=0.008),while no significant statistically difference (P>0.013) was found between the later two groups.Conclusions Gastroscopy can be used as a basic examination means for GERD;short segment hiatal hernia can be regarded as an early form of hiatal hernia,and is of important reference value for the diagnosis and treatment of GERD;more serious hiatal hernia and esophagitis could be found in elderly GERD patients.

ABSTRACT

Objective To produce three-dimensional cartilage nanoscaffolds based on extracellular matrix.Methods Nanoscaffolds of collagen type Ⅱ(Col-Ⅱ), hyaluronic acid(HA)and chondroitin sulfate(CS)were prepared by mixing water,trifluoroethanol and hexafluoroisopropanol as a solvent.The structure, morphology, thermal property, mechanical performance and hydrophobicity of the scaffolds were characterized.Results There were interactions between Col-Ⅱ,HA and CS.The scaffolds were hydrophobic.The Col-Ⅱ triple-helix structure wasn't completely damaged.In the study, scaffold fibers were smooth,slender and dimensionally stable.The scaffolds had good thermal stability and optimal tensile properties could be obtained at the mass ratio of 7:1:1.Conclusion In this study, scaffolds have good thermal, mechanical and structural properties and are expected to be used in cartilage repair.

ABSTRACT

The response rules of pressing pain on the back section in the Governor Vessel in patients with gastro-esophageal reflux disease (GERD) were studied to provide references for the diagnosis and treatment of GERD. Seventy-six cases of GERD were included into an observation group while 30 healthy volunteers were recruited into a control group. A mechanical measurement device of pressing pain that could measure the pain threshold was adapted to observe the pressing pain on the back section in the Governor Vessel in GERD patients and healthy volunteers. The test area is from spinous process of the 1st thoracic vertebra to that of the 12th thoracic vertebra (T1 -T12), including acupoints and non-acupoints on the Governor Vessel. As a result, in the observation group the pain threshold of T5-T7 spinous process clearance, which was the location of Shendao (GV 11), Lingtai (GV 10) and Zhiyang (GV 9), was lower than that in the control group (all P < 0.05). This result indicated that there was significant pressing pain in T5-T7 spinous process clearance in patients with GERD, which could be taken as an important auxiliary diagnosis and a new thinking method in the treatment of GERD with acupuncture.

ABSTRACT

<p><b>BACKGROUND</b>Diagnosis and treatment for respiratory symptoms (RSs) of gastroesophageal reflux disease (GERD) is more difficult than that for common esophageal symptoms. The goal of this study was to evaluate the efficacy and safety of radiofrequency (RF) treatment on RSs of GERD in a preliminary 12-month follow-up observation.</p><p><b>METHODS</b>From April 2006 to October 2008, 505 GERD patients with mainly respiratory presentations such as wheezing, chronic cough or hoarseness, were treated by endoscopic RF. A questionnaire was completed before and after treatment, using a six-point scale ranging from 0 to 5 to assess symptom severity and frequency. The symptom score was the sum of frequency and severity.</p><p><b>RESULTS</b>Symptom scores were significantly improved at the end of the follow-up period. The mean heartburn score decreased from 5.31 to 1.79. The mean regurgitation score decreased from 5.02 to 1.64; mean cough score decreased from 6.77 to 2.85; mean wheezing score decreased from 7.83 to 3.07; and mean hoarseness score decreased from 5.13 to 1.81 (P < 0.01). No major complications or deaths occurred. Minor complications included temporary post-procedural retrosternal unease or pain (n = 106; 21.0%), mild fever (n = 86; 17.0%), transient nausea/vomiting (n = 97; 19.2%), and transient dysphagia (n = 42; 9.3%). Thirty-five (6.9%) patients had recurrence of symptoms. Endoscopic RF treatment was repeated in six patients, and laparoscopic fundoplication was performed in seven.</p><p><b>CONCLUSION</b>Endoscopic RF is an effective and safe means to treat RSs in patients with GERD.</p>

Objective To evaluate the cellular toxic and release of pharmaceutical dressing. Methods Following the State standard GB/T14233.2-2005, the L929 cellular morphology was observed by inverted microscopy after 72h and proliferation of the cells was examined using mitochondrial function (MTT) assay. Relative growth rate (RGR) was calculated and cytotoxicity grade was evaluated. With PBS7.4 as dissolution media, and (32±0.5)℃ as dissolution temperature, the release rate was determined with UV method with the determination wavelength of 288nm and the dissolved liquid in1/6, 1/2, 1, 3, 16, 24, 36 and48h. Results The average cell RGR of the pharmaceutical dressing was 91.25% and reached 1 grade. L929 cellular morphology was normal. Pharmaceutical dressing release accord to Higuchi equation,and the simulated equation is Mt/M∞=0.3271t0.239. Conclusion Biologic compatibility of the pharmaceutical dressing is good, and the release of levofloxacin from the pharmaceutical dressing is sustained in vitro.

ABSTRACT

<p><b>BACKGROUND</b>Gastroesophageal reflux disease with extra-esophageal symptoms, especially those with respiratory distress was attracting more and more attention. The related mechanisms were still in controversy. The purpose of the work was to explore airway inflammation triggered by gastroesophageal reflux.</p><p><b>METHODS</b>Sixteen Sprague-Dawley rats were used as study group and 9 as control. In the study group, a plastic extender with a trumpet-shaped distal end was inserted into the lower esophagus to dilate the cardia, the pylorus was ligated. One ml of 0.1 mol/L hydrochloric acid was injected into the stomach. While a simple laparotomy was performed for control animals. All animals from two groups were sacrificed 24 hours after operation. Then tracheotomy was carried and the bronchoalveolar lavage fluid was collected in all animals. Cells in the fluid were counted and levels of interleukin (IL)-5, -6, -8 in it were measured.</p><p><b>RESULTS</b>Compared with control group, the study group presented a neutrophil pattern of airway inflammation and an elevated concentration of IL-5, -6, -8 with no significant difference regarding eosinophil count.</p><p><b>CONCLUSION</b>The gastroesophageal reflux-triggered airway inflammation is characterized by a neutrophilic airway inflammation which differed from that caused by asthma, and enhanced levels of IL-5, -6 and -8, which are similar to that caused by asthma.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the occurrence of delayed neuronal degeneration, activation of microglia and nuclear factor-kappa B after rat intracerebral hemorrhage (ICH) and the possible role of iron.</p><p><b>METHODS</b>ICH model was induced by infusion of autologous whole blood into the right basal ganglia. To evaluate the possible role of iron on delayed neuron loss, an iron model by injection of FeCl(2) into hippocampus was also set up. Degeneration of neurons and the activation of microglia and NF-kappa B were detected.</p><p><b>RESULT</b>Both whole blood and iron caused neuron degeneration for at least 14 days were revealed by Fluoro-jade C staining. Consistently, activated microglia and NF-kappa B positive cells were also observed in the peri-hematoma area and the ipsilateral hippocampus.</p><p><b>CONCLUSION</b>The iron may participate in the delayed neuron injury followed ICH; the activated microglia and NF-kappa B may be involved in the process of delayed neuronal injury.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effects of Xuezhikang and simvastatin on cerebral ischemia/reperfusion injury in rat, as well as the influences after intervention with L-NAME.</p><p><b>METHOD</b>Rats were given orally with Xuezhikang and simvastatin or vehicle for 2 weeks, and then subjected to middle cerebral artery occlusion for 120 min using intraluminal filament model. L-NAME were injected into the lateral ventricles in half of the rats treated with Xuezhikang and simvastatin 45 min before the ischemia. The neurological deficits examinations were performed at 2, 24, 48 h after reperfusion. After the last examination the animals were sacrificed, the infarct volumes were determined by TTC staining, and MDA levels were also measured.</p><p><b>RESULT</b>Xuezhikang and simvastatin both significantly reduced the infarct volume and improved the functional recovery when compared to vehicle. Xuezhikang and simvastatin both significantly decreased the MDA accumulation after reperfusion. L-NAME partially inhibited the protective effect of Xuezhikang but nearly completely abolished the protective effect of simvastatin.</p><p><b>CONCLUSION</b>Xuezhikang has protective effects on ischemic brain damage in rats, which the beneficial effects are partly due to the statins components. The other components in Xuezhikang may also account for the neuroprotective effects, which is worth further investigations.</p>

ABSTRACT

MTT Colorimetric method is usually applied for measuring the living animal cell number. By changing the reaction temperature and the reaction time as well as the colorimetric wavelength, the improved MTT colorimetric method was established to count the living bacterial cell number. This new method was used to measure the living cell concentration in the process for culturing bacteria PBW1. The results measured by the improved MIT colorimetric method and dilute plate method are similar. Compared with other methods including the dilute plate method, the improved MTT colorimetric method has many advantages such as accuracy, quickness.
